Our care managers conduct a detailed bio-psycho-social assessment of the client(s). The investigation is specific to the situation of the elder, their current condition, and the types of services being considered.

  • Assess activities of daily living, intellectual functioning, decision making capacity, and emotional status
  • Evaluate social interactions, isolation, recent changes and conflicts which affect quality of life
  • Investigate mental and emotional health, current outlook, and effects on physical health
  • Check legal documents are consistent with current situation, identify conflicts, and check for appropriateness
  • Inspect the home environment for safety, security and consistency with elder's current and future needs
  • Evaluate medical condition and health care needs including in-home and in-patient care

After consulting with Mary, Tina visits Barbara to perform a detailed assessment of her health, living situation, and mental status. She also obtains consent from Barbara to discuss her condition and medications with her physician and physical therapist.

Tina finds that Barbara has been taking her high blood pressure medication sporadically and is having trouble using her weekly pill container. She suggests this could be contributory to her falls. She also suspects that Barbara may have a slight hearing loss as she noticed Barbara missed a telephone ringing during her visit.

Barbara also is having trouble with her meal preparation. Tina finds Barbara has placed a container of ice cream in the butter tray. She also finds delinquent utility and tax bills of which Barbara was unaware.

She finds Barbara has only limited social contacts because she stopped driving after her first fall. She desires to attend local church functions more often but has been unable to find alternative methods of transportation.

Tina prepares a detailed written report containing these and other bio-psycho-social factors she has uncovered in her visit. Her report also explains the details of each assessment factor so that Mary and Barbara can easily understand. <Next>
