Holistic Living Plan

The holistic living plan is a roadmap for the near term and future identifying options for the elder and their family.

 

  • Geared toward keeping or returning the elder to the optimum level of independence and stability
  • Specifies local resources, programs, services, and supports required to meet the needs identified in the assessment
  • Identifies procedures to be used in an emergency
  • Based on assessment and involves elder, family and care manager
  • Focuses on types of assistance the elder needs to optimally function and remain independent as possible
  • Goal is to be accepted by elder and family as the roadmap for care
  • Considers near term options as well as longer term strategies
  • Includes treatment options, referrals, and estimates of resources required
  • Includes options in all areas of elder's environment
  • Details for placement options in retirement centers, assisted living or skilled nursing facilities

Tina prepares a written holistic living plan based on her assessment of each bio-psycho-social factor.

Physical: Tina suggests having a Lifeline system installed at Barbara's house to allow her to call for assistance in case of an emergency. She recommends that her physician follow up with a neurological exam to eliminate vascular dementia or other physical problems that could have contributed to Barbara's falls. She proposes to work with Barbara's physical therapist and physician for follow-on strength and balance training after physical therapy ends, and on Barbara's hearing loss and need for a hearing aid. Tina gives referrals to three bonded agencies that can provide a companion to help with cooking and transportation several times a week. Tina provides information on senior transportation options and Meals on Wheels that Barbara can access. Tina does not make a recommendation for in-home health care at this point.

Psychological: Tina recommends that Barbara's primary care physician refer her to a psychologist to perform a neuro-psychological evaluation to identify possible causes for her falls such as visual-spatial deficit. She refers Barbara to a reminiscence/life review support group meeting weekly in Carmel to address Barbara's isolation, depression and work on her lack of confidence.

Social: Tina provides the names of two Adult Day Care centers in Barbara's area that provide pick/drop off service. She meets with the pastor of Barbara's church to identify resources within the congregation to help with Barbara's transportation to church functions. She suggests that Barbara attend a weekly woman's circle at the church. Tina proposes to set up email access for Barbara and educate her on how to maintain email contact with her daughter, grandchildren and family.

Legal / Financial: Tina recommends that Barbara establish a durable power of attorney for both finances and health care and provides the name of two elder law attorneys in the area who can set these up. She also suggests that Barbara have the attorney review her will and estate plans. She recommends a local accountant to assist Barbara in bill paying. She proposes to work with Mary to determine Barbara's insurance benefits and entitlements under social security and Medicare and to assist Barbara and Mary in filing claims.

Long term plan: Tina provides long-term cost projections for several living settings in the Carmel area based on Barbara's assets, entitlements, and future physical needs. She includes cost projections for geriatric care management for Barbara as well. She suggests working with Mary and Barbara to determine a strategy for optimum use of assets and entitlements in the future with which everyone is comfortable. Tina also proposes to assist them in end of life planning. <Next>
