When evaluating a geriatric care management practice for yourself or a loved one, it is very important that you ask questions to determine the qualifications of the care manager you select.
Selection Criteria

LICENSURE/CERTIFICATION:
While not all care managers hold a license in their profession, does my care manager hold a California license to practice?

Does my care manager hold the California LCSW (Licensed Clinical Social Worker) or RN, MSN (Registered Nurse, Master's Degree) license?
Does my care manager have or are they in the process of obtaining CCM (Certified Case Manager) certification?
EXPERIENCE / KNOWLEDGE:
Does my care manager have at least five years experience providing care management in independent practice or with an agency?
Does my care management practice have managers who are experienced health care and business professionals?
Does my care manager know all the local resources that might be required by the client?
Does my care manager have first hand knowledge of local retirement communities, assisted living, skilled care facilities and hospitals?
Does my care manager have knowledge of geriatric physicians in the community including their availability and types of insurance accepted?
Does my care manager have knowledge of Medicare / Medi-Cal provisions and can they plan for optimum use of entitlements?
SERVICES:
Is my care manager available on a 24x7x365 basis for emergencies?
Is my care manager independent of a business interest in selection or referral of services or agencies?
Is my care manager a single point of contact for family, elders and service providers?
Will my care manager perform an independent, bio-psycho-social assessment?
Will my care manager provide a written living plan that presents near-term and future care strategies along with cost estimates?
Will my care manager make frequent home visits to continuously monitor health care delivery and the living situation?
Will my care manager have periodic interdisciplinary team meetings evaluating status and ways to improve quality of life?
Can my care manager refer or arrange the following services:  

In-home aids

Companion / homemakers

Nursing services

Therapy services

Social services

Transportation

Meal services

Grocery delivery

Medical equipment

House cleaning

Home repairs

Lifeline installation

Internet connection and training

High technology home monitoring
