We'll implement the options you select in the living plan by referring the best care suppliers and overseeing their work, making arrangements for all services, coordinating, facilitating and implementing to quickly realize positive changes in the elder's quality of life.

  • Make arrangements with facilities, caregivers, physicians, therapists, and other professionals to achieve the goals of the plan
  • Coordinate with agencies and staff to maximize the quality and efficiency of care
  • Facilitate communication between elder, family and physcians, and care givers
  • Help file insurance forms, assist with record keeping and bill payment
  • Assist in transition to alternate living facilties
  • Provide a local surrogate for a family living remotely
  • Facilitate expedited delivery of health equipment
  • Oversee installation and monitor Lifeline emergency notification equipment
  • Set up Internet access for elders to increase communication with family and friends

Tina facilitates a meeting with Barbara and Mary to review the living plan and get agreement on the next steps.

During this meeting, Barbara agrees to pursue a thorough neurological evaluation. Mary asks Tina to set up an appointment with Barbara's primary care physician and accompany Barbara to the appointment. She will see that Barbara has transportation to other specialist appointments and will accompany her if Barbara requests.

Barbara agrees to try the Adult Day Care in Carmel and Tina will arrange the enrollment. She also agrees to have a Lifeline system installed and Tina will also see that this is accomplished. Tina will be on the call list in case of an emergency.

They agree to reevaluate the need for a home companion in two months, but for now Barbara doesn't want a "stranger" in her house. Tina will refer Barbara to Meals on Wheels and she will explain their service to her.

At Barbara's request, Tina sets up an appointment with an elder law attorney and elder accountant and Tina will accompany Barbara to these meetings. Tina will review the long term care strategy with Mary and Barbara after the meeting with the attorneys and Barbara will get on a waiting list with one of the local retirement centers if necessary.

Tina will have an email system set up using Barbara's home television and teach Barbara how to communicate with her family and friends.

Tina documents the meeting and sends a record of their plans and actions to Mary and Barbara. Tina will visit Barbara frequently during the implementation period and will have weekly phone calls with Mary. She will also provide a written status report to Mary. <Next>
